Waiver
PARTICIPANT WAIVER AND RELEASE OF LIABILITY
This waiver must be completed and signed by a parent or legal guardian prior to any child participating in activities at Tiny Town Adventurez.
I acknowledge and agree to the following on behalf of myself and my child(ren):

Assumption of Risk
I understand that participation in indoor play activities, including soft play, imaginative play, sensory experiences, and parties, involves inherent risks. These may include, but are not limited to, minor injuries such as bumps, bruises, and scrapes. I voluntarily assume all risks associated with my child(ren)'s participation. -
Supervision and Responsibility
I understand that Tiny Town Adventurez is a parent-supervised facility. I agree to remain on-site and supervise my child(ren) at all times unless attending a private, staff-led party. I am responsible for ensuring my child(ren) follow all rules and safety guidelines. -
Health and Wellness Policy
I affirm that my child(ren) and I are free of contagious illness, including but not limited to COVID-19, flu, fever, rash, vomiting, or diarrhea within the last 24 hours. I agree not to attend if anyone in my party exhibits symptoms of illness. -
Rules and Conduct
I understand that socks are required in all play areas. I will ensure my child(ren) play safely, respectfully, and follow directions from Tiny Town Adventurez staff. I understand that disruptive or dangerous behavior may result in removal from the facility without refund. -
Liability Release
I release and hold harmless Tiny Town Adventurez LLC, its owners, staff, agents, and affiliates from any and all liability, claims, demands, injuries, or damages arising from participation in activities at the facility, even if caused by negligence. -
Medical Treatment
In the event of injury or emergency, I authorize Tiny Town Adventurez staff to seek basic first aid or emergency medical care for my child(ren). I understand I will be financially responsible for any such care. -
Acknowledgement of Policies
I have read, understand, and agree to comply with Tiny Town Adventurez's posted rules, safety signage, and cancellation policies.
